Add optional description text for each field

Description

Splitting the request from T367223#9904871 to add support for descriptions above the field as opposed to the help texts which are already support but are displayed below the field. This is useful to be able to give hints before filing a field.

Acceptance criteria

  • A optional -description message is added for each field
